Oxford classification of strokes
TACS -
- All 3 of: Unilateral hemiparesis, homonymous Hemianopia, higher cortical dysfunction (speach or visuospatial)
- Large strokes, MCA/ICA thrombus/embolus
PACS -
- 2 of: Unilateral hemiparesis, homonymous Hemianopia, higher cortical dysfunction (speach or visuospatial)
- Tend to have a stroke affecting a branch of the MCA thrombus/embolus
POCS -
- 1 of:
- Bilateral motor or sensory deficit
- Cerebellar/brainstem signs
- Isolated homonymous hemianopia
- conjugate eye movement disorder
- Cranial nerve palsy & contralateral motor/sensory deficit
- Vertebrobasillar arteries: Brain stem, thalamus, cerebellum, occipital lobes
LACS -
-
- ataxic hemiparesis 2) pure motor 3) pure sensory 4) sensorimotor 5) dysarthria + clumsy hand
- Due to occlusion of small vessels, end arteries so area being supplied dies
- Basal ganglia, thalamus, internal capsule, brain stem
Posterior stroke syndromes
- Basilar artery occlusion
- Anterior inferior cerebellar artery occlusion
- Wallenberg’s syndrome (lateral medullary syndrome)
- DANVAH
- dysphagia
- ipsilateral ataxia
- ipsilateral nystagmus
- vertigo
- anasthesia
- ipsilateral horners syndrome
- Weber’s syndrome (medial midbrain syndrome) - (paramedian branches of the upper basilar and proximal posterior cerebral arteries)
