A keloid is an abnormal form of scarring (a fibroproliferative disorder); it is not associated with maturation, regression, or contraction.
- Excessive scar formation beyong borders of original wound
Risk factors:
- History or prior keloid
- darker skin types
PC
- Erythematous, smooth and shiny elavated scar with overhanging edge
- Slow growing
- Hx of inciting event - body piercing, surgery, insect bite, vaccination
- common sites (in order of decreasing frequency): sternum, shoulder, neck, face, extensor surface of limbs, trunk
Investigations
*Usually a clinical diagnosis
- Consider a biopsy if diagnosis in doubt (?dermatofibrosarcoma)
Management
- early keloids may be treated with intra-lesional steroids e.g. triamcinolone
- excision is sometimes required but careful consideration needs to given to the potential to create further keloid scarring
