An extremely rare disease

Goodpasture’s disease - Also known as anti-glomerular basement membrane antibody (anti-GBM) disease. Caused by an antibody to the alpha-3 chain of type IV collagen (this collagen is found in the basement membranes of alveoli and glomeruli.

[anti-GBM glomerulonephritis] - when it affects only the kidneys

Good pasture syndrome (pulmonary renal syndrome) - Consists of:

• Glomerulonephritis (nephritic syndrome)
• Pulmonary haemorrhage

*Main differential of acute renal failure + haemoptysis - Wegener’s granulomatosis

Risk factors:

• HLA-DRB1 or DR4 - More then 90% of patients have this gene

PC

• Reduced urine output
• Haematuria
• Haemoptysis (in 60% of patients)
• Oedema (in 34% of patients)
• SOB, cough fever (in 28% of patients)

On examinations - Crackles in lung bases
